Light-Based Therapy: A Promising Option for Alopecia Areata

If you suddenly notice a smooth, round bald spot on your scalp, it can be frightening. One possible cause is a condition called alopecia areata. The good news is that the hair follicle is often still there — and new treatments may help give it a better chance to grow hair again.

What Is Alopecia Areata?
Alopecia areata is a type of autoimmune hair loss. Here is an easy way to understand it:
Your immune system is supposed to protect your body from things that can make you sick. But with alopecia areata, the immune system becomes confused and attacks healthy hair follicles by mistake.
Think of each hair follicle as a tiny hair-growing factory. With alopecia areata, the immune system sends the wrong message to that factory and tells it to slow down or stop working. This can cause hair to fall out, often leaving smooth, round or oval patches. The follicle is usually not destroyed. That is important because it means the follicle may still have the ability to grow hair again.
How Can Light-Based Therapy Help?
One light-based treatment that has been studied for alopecia areata is called 308-nm excimer light or excimer laser therapy.
This treatment sends a very specific wavelength of light directly to the areas affected by alopecia areata.
The goal is not simply to “shine a light on the hair.” The light works within the skin and may help calm the immune activity that is interfering with the hair follicle. When that immune response becomes quieter, the follicle may have a better opportunity to return to its normal growth cycle. Research has shown that 308-nm excimer treatment can improve hair regrowth in some people with alopecia areata. A systematic review found that excimer light treatment performed better than untreated areas, although more research is still needed.
An Easy Way to Think About It
Imagine the hair follicle is a plant. The plant is still alive, but something around it is preventing it from growing normally. Light-based therapy does not simply “make hair grow.” Instead, the goal is to help improve the environment around the follicle so that it may have a better chance to begin growing again.
That difference is important.
Who May Benefit?
Light-based treatment may be most useful for some people who have smaller areas of patchy alopecia areata, particularly on the scalp. Studies have reported hair regrowth with 308-nm excimer therapy, but results vary from person to person. In one randomized study, both excimer laser and corticosteroid injections produced regrowth, although the injections produced a stronger response during the treatment period. That is why there is no single treatment that is right for every person with alopecia areata.
Hair Loss Should Be Evaluated Before It Is Treated
Not every bald spot is alopecia areata.
Hair loss may also be connected to:
Female or male pattern hair loss
Telogen effluvium or excessive shedding
Traction alopecia
Scalp inflammation
Scarring alopecia
Nutritional deficiencies
Hormonal changes
Medication-related hair loss
Other medical conditions
